Well Methadone is a C2 controlled drug and very effective in treating moderate to severe pain. There are many other drugs in that category that your doctor can choose from such as Oxycodone and Morphine. The dose would be assessed and raised slowly and gradually to avoid affecting your breathing. With any opiate type treatment after you've taken it for while then your body adapts to it. So the solution is when the dose you currently take doesn't cover your pain any longer then inform your doctor and he/she will adjust your dose upwardds or switch you to another long acting drug along with it's shorter acting version for breakthrough pain.
